In the Home -- Asthma Community Health Worker (CHW) Program

What is the CHW Program? 

LBACA’s CHW program is a team approach to asthma management, which involves parent, child, doctor, school after-school program, child-care, coach, and anyone that spends time caring for the child.  Our target areas are the cities of Long Beach, Wilmington, Carson, and San Pedro.  The CHWs educate and advocate for parents of children who have moderate to severe persistent asthma and are poorly controlled.  In order to be eligible to participate families:

  • Must have a Primary Care Physician (PCP)

  • Live in Long Beach, Wilmington, Carson or San Pedro

  • Age: under 18

  • Recent uncontrolled asthma defined by:

    • Missed school days due to asthma

    • Emergency room visits for asthma

    • hospitalizations for asthma

    • Acute clinical or doctor's office visits for asthma

Providers, public health nurses, school nurses, hospitals, emergency room departments, Asthma Resource Center, and self-referrals provide referrals to our Asthma CHW program.

What training do the CHWs receive?

The 5 Community Health Workers were trained in the YES WE CAN Model developed by California State University, San Francisco and the City College of San Francisco, which is used to train Community Health Workers nation-wide.  CHWs received over 40 hours of training and continue to receive ongoing training.  They were trained on effective communication, home visits, handling difficult situations, All About Asthma (signs, symptoms, triggers, medications, gadget use, Asthma Action Plans), Evaluation (purpose, probing questions, keeping participants engaged), Safety, Personal Defense, Asset Mapping of these target areas, Integrated Pest Management, Lead and its health impacts, CPR training, and Housing Rights and Tenants Issues.

What do the CHWs do? 

Our CHWs provide 3 home visits, perform home environment assessments to identify what might trigger a child’s asthma attack, educate families to improve their child’s asthma, provide free supplies to minimize household triggers, provide advocacy with landlords and providers, and link families to appropriate services.
